In subject line: Daniel Johnson, A1234BC, DOB 08/03/2000, Urgent: not receiving bipolar medication
For the attention of the healthcare department,
I am writing this e-mail on behalf of Daniel Johnson, prisoner number A1234BC, DOB 08/03/2000.
My name is Jane Johnson, Daniel’s mother and carer since he was 15.
- Daniel has a diagnosis of bipolar disorder, and since 15/07/2024 he has not been receiving his daily lamotrigine tablets that help him to manage his symptoms.
- Daniel has taken lamotrigine to manage his bipolar symptoms and to keep him calm for the past 5 years. He continued to receive his prescription in prison until 3 days ago (15/07/2024) when his medication stopped.
- He was not told why, and he is worried that not taking his medication will negatively impact his mental health.
When Daniel first came to prison in November 2023, I posted a copy of his medical records to the prison on 25/11/2023. I am happy to send these documents again if they were not received or if you require evidence of his diagnosis.
As his mother, I am worried about his well-being and what he might do to himself without his medication.
I would appreciate it if the healthcare department could investigate this issue. Please could they make sure that Daniel’s lamotrigine prescription is still in place and ensure he gets his tablets daily.
I’d also like to request that they please ask Daniel if he consents to sharing his medical information with me, so that I can be kept in the loop about this issue.
I can be reached from 8:00am to 8:00pm Monday to Thursday and any time from Friday through Sunday on 07111 222333. You can also e-mail me at jane.johnson123@gmail.com.
Thank you for your help.
Sincerely,
Jane Johnson
